How to register names of martyrs and correct information

The draft list of names of martyrs in student uprising collected from various public and private hospitals and district level in anti-discrimination student movement is available on Ministry of Health and Family Welfare Health Service Department website: www.hsd.gov.bd and Department of Health website: www.dghs.gov. bd is published.

The list published on the website will be open to the public from September 23 to October 6 for correction and addition of information.

The data released on Thursday (September 26) stated in the statement. It has been said that the family members, heirs and representatives of the martyrs have been requested to verify, correct and complete the names, addresses and other relevant information published in the list.

If anyone has any opinion, suggestion and any new information to be added regarding the published list, it is requested to inform the concerned hospital or the nearest government hospital. The following steps should be taken to correct the information in the published list:

a) Martyr’s family members, heirs and representatives should register with National Identity Card (NID)/birth certificate and mobile number.

b) After registration the details of the martyr should be downloaded and printed.

c) The blank spaces of the printed paper should be filled.

d) The concerned institution will assist in correcting the information after going to the concerned hospital or the nearest government hospital with the filled information.

e) The person in charge of the institution will update the information online by submitting the form filled by the family members, heirs and representatives of the martyr.

f) The submitted information can be re-verified by accessing the website whether it has been properly entered or amended.

If anyone’s name is not included in the list of martyrs of the recent anti-discrimination student movement, then the family members, heirs and representatives of the martyrs are requested to contact the concerned District Administrator, Civil Surgeon and Upazila Executive Officer or Upazila Health and Family Planning Officer with suitable evidence.
